NON PLANNING SERVICES:
An initial retainer of $100 and a signed contract must be secured prior to any services being performed.
This deposit is nonrefundable and non-transferable. This deposit will be put toward the client’s total balance.
Contracted package
Consultations are not required but recommended and subject to travel fees. Any additional services/products requested by the client incurred by consultant will be billed directly to client at cost and mileage at $.55 per mile if extensive travel (beyond 25 miles) is required to acquire items.
Expiration
Current event date is valid 1-week post origination date to secure services.
Provided Documentation
All documentation created and provided to client is owned and copy written by Consultant and cannot be used or given to any other 3rd party.
Attire
All associates of Consultant will wear proper attire day of event. Day of Management/Consultant – Will be a Lead Associate and team member of Consultant who will be present and manage day of activities.
Liability
It is understood that Consultant is only liable for services completed and provided by Consultant and cannot be held liable for the services of other contracted vendors. It is understood that in no event shall Consultant be liable for consequential damages of any kind.
All brushes, tools, and products are sanitized between every makeup application. Makeup products used are hypoallergenic. Any allergies and/or skin conditions should be reported by the client to the Consultant prior to application and, if need be, a sample test of makeup may be performed on the skin to test reaction.
Cancellation Policy
APPOINTMENTS: Due to the nature of the session, a strict timeline will be enforced to ensure all obligations are met. The session will start at the scheduled start time even if the client is tardy or unprepared. If the schedule exceeds the allotted time due to client delays, extra time will be charge at $65 per hour. If additional time is required, payment is due in full on the date of the appointment.
If it is necessary to reschedule, we require 48 hours’ advanced notice of your scheduled appointment to avoid cancellation/rescheduling fees. Exceptions to this policy will be considered on a case by case basis.
Cancellations must be made (30) days prior to the client’s reserved date or the client will be responsible for paying the full amount of services agreed upon in this contract. No portion of the fees paid to Consultant will be returned. If the event is postponed, services can be applied to a new date if available.
Responsibility
Consultant is not responsible for Acts Of God, Natural Disasters, an Act Of Government such as A Declaration of National or Local Emergency, or Other Incidents Not within the control of the consultant, i.e. accident, death in the family, illness, pregnancy, or sudden tragic circumstance. In such a situation, consultant will obtain, upon approval of the client, a qualified professional replacement to fulfill consultant’s obligations under the contract, at no additional charge to the client.
Contracted Vendor and Payments
Any rentals and/or subcontracted vendors contracted on behalf of client will be paid in full to Consultant directly at requested deadline. Proper notice will be provided per cost.
Damage or Loss of Accessories
Client agrees to exercise all due care in caring for, and preserving the property of Consultant. Clients shall remain responsible for all loss or damage to rentals, up to and including actual replacement value for each missing or damaged item per cost.
Images
Unless prior discussed, the Client grants Collectively Chic Boutique and Events unrestricted usage rights and copyrights to use and publish photographs in any manner or medium. (Images may be used on websites, social media and / or in any brochure, flyer, ext.)
Payment Schedule and Method
For your convenience, payments can be made via Venmo using a valid credit card. Otherwise, payment is to be made by cash or check. $100 initial retainer due upon completion of the signed agreement for services. The final balance is due before the Consultant departs — no exceptions. The person(s) responsible for the entire balance of payment is the person(s) whose name(s) appear on the contract. Gratuity is never expected but always appreciated. (NOTE: bridal consultations, travel expenses and retainer fees are separate costs.)
AMENDMENTS TO SERVICES: Any additional add on's or additional coverage needed must be made known at least 2 week prior to contract date.

This deposit will be put toward the client’s total balance. The second installment is due 90 days prior to event start date. The remaining balance will be due 30 days before the event start date.

Include all services described in selected package details reviewed and distributed to clients. Any additional services/products requested by the client incurred by consultant will be billed directly to client at cost. All services contracted cannot be deducted, but additional services can be added.

Travel
Travel within 60 miles (round trip) of Osage Beach, Missouri is free of charge. For events outside of the limit, $0.55 will be charged per mile for travel expenses. A $250 flat rate travel fee is added if travel is 2+ hours or out of state. In the event that the client specifies a certain location to be used, the client is responsible for all location fees involved such as location access fees, parking, etc. Client is responsible for arranging the location access and any permits/passes related to doing so. If the event goes late into the evening the client is responsible for overnight accommodations and subject to additional costs. Hotel accommodations are required within 10 – 15 miles of site in a safe location for all employees of Consultant working the events for any events located further than 90 miles away. The client will provide food during the event if services last more than six (6) hours.
For all events deemed as destination (taking place beyond 125 miles), additional fees are applicable, such as round trip airfare, overnight accommodations for a minimum of two nights (Consultant will arrive the day before the event or as early as possible in case of any flight issues and stay overnight the night of the event), meals, transportation arrangements from airport and back during the stay. Rental cars, cab fees, airport parking fees etc are discussed with the client pre travel to discuss a reasonable finance plan. All travel can be arranged by clients per specifications of Consultant or arranged by Consultant with agreement of payment invoiced. Should Consultant choose to extend the trip there is no additional fee for the client.

If event is postponed, all services will cease at time of notification of postponement if no determined new date is provided. Payment schedule will be determined and readjusted post new event date notification. If client requires services to resume, post new event date being established, remaining balance will be required at time of service re-start.

Overtime
Additional unplanned and/or un-contracted services (such as, but not limited to, full redo of design, day of event extension of time, services outside of scope of agreement, management of non wedding day activities, etc) are considered overtime and will be billed with prior notification and consent at the rate of $130 per hour.

Attorney
If legal action is necessary to enforce the terms of this contract the prevailing party shall be entitled to reasonable attorney fees in additional to any other remedies to which that party might be legally entitled.
Appropriate Conduct/ Safe Working Environment:
The Client(s) expressly agree(s) to take best efforts to provide Collectively Chic Boutique and Events staff with a safe and professional relationship. In the event of circumstances deemed to present a threat or implied threat of injury or unprofessional, harassing language or behavior, Consultant reserves the right to cancel all services remaining under this Agreement and leave the event and the Client(s) will agree to relieve and hold Consultant harmless as a result of incomplete services under this Agreement, or for a lapse in the quality of the Consultants work. The Client(s) shall be responsible for payment in full, regardless of whether the situation is resolved or whether Consultnat resumes work detailed in this Agreement.
The undersigned have read this contract, understand its terms, and agree to be bound thereby. Any additions, deletions, or revisions must be made in writing and approved by all responsible parties. The parties agree that this contract is the complete and exclusive statement of the mutual understanding of the parties.
